What accounts for poor functioning in people with schizophrenia: a re-evaluation of the contributions of neurocognitive v. attitudinal and motivational factors

Background
Neurocognitive deficits are often seen as core features of schizophrenia, and as primary determinants of poor functioning. Yet, our clinical observations suggest that individuals who score within the impaired range on standardized tests can reliably perform better in complex real-world situations, especially when performance is embedded within a positive socio-affective context.
MethodsWe analyzed literature on the influence of non-neurocognitive factors on test performance in order to clarify their contributions.
ResultsWe identified seven non-neurocognitive factors that significantly contribute to neurocognitive test performance: avolition, dysfunctional attitudes, effort, stress, negative emotions, asociality, and disorganized symptoms. We then proposed an alternative model based on dysfunctional (e.g. defeatist) attitudes and their consequences for motivation and sustained task engagement. We demonstrated that these factors account for substantial variance in negative symptoms, neurocognitive test performance, and functional outcomes. We then demonstrated that recovery-oriented cognitive therapy – which is derived from this alternative model and primarily targets dysfunctional beliefs – has been successful in the treatment of low functioning individuals with schizophrenia.
ConclusionThe contributions of neurocognitive impairments to poor real-world functioning in people with schizophrenia may be overstated in the literature, and may even be limited relative to non-neurocognitive factors. We offer suggestions for further research to more precisely quantify the contributions of attitudinal/motivation v. neurocognitive factors in schizophrenia.

The Social Calculus of Voting: Interpersonal, Media, and Organizational Influences on Presidential Choices

Voting choices are a product of both personal attitudes and social contexts, of a personal and a social calculus. Research has illuminated the personal calculus of voting, but the social calculus has received little attention since the 1940s. This study expands our understanding of the social influences on individual choice by examining the relationship of partisan biases in media, organizational, and interpersonal intermediaries to the voting choices of Americans. Its results show that the traditional sources of social influence still dominate: Interpersonal discussion outweighs the media in affecting the vote. Media effects appear to be the product of newspaper editorial pages rather than television or newspaper reporting, which contain so little perceptible bias that they often are misperceived as hostile. Parties and secondary organizations also are influential, but only for less interested voters—who are more affected by social contexts in general. Overall, this study demonstrates that democratic citizens are embedded in social contexts that join with personal traits in shaping their voting decisions.

Objective:
To better define the role of multiple risk factors for cytotoxic Clostridium difficile-associated diarrhea.
Design:Case-control study.
Setting:A Veterans Affairs Medical Center.
Patients:Thirty-three case patients with C difficile -associated diarrhea. Two control groups were used: one group consisted of 32 patients from the same ward as the case patients, and one group consisted of 34 patients with nosocomial diarrhea and negative C difficile toxin assays.
Intervention:None.
Results:Multivariate analyses revealed that exposure to second- or third-generation cepha-losporins was the most important independent risk factor, even after controlling for other antimicrobial use (odds ratio [OR]=8.3, 95% confidence interval [CI95] = 1.4 to 48.9 compared to ward controls; OR= 9.6, CI95 = 2.1 to 44.1 compared with diarrhea controls). Persons exposed to two or more antimicrobials simultaneously were at substantially elevated risk (OR= 18.7, CI95 = 4.1 to 85.8 compared with ward controls; OR=21.5, CI95=3.2 to 141.9 compared with diarrhea controls).
Conclusion:Physicians should consider carefully the appropriateness of second- and third-generation cephalosporin use and combination antimicrobial therapy, especially during nosocomial C difficile -associated diarrhea outbreaks.

The purpose of this paper was to review the methodological rigor and clinical implications of behavior modification studies in which social skills have been taught to socially deficient children. Thirty two studies were reviewed. The following four treatment approaches were identified and reviewed: adult contingent attention; peer-mediated strategies; modeling; and treatment packages. The clinical merits and limitation of each approach are discussed. Identification and possible target behaviors for neglected and rejected children are outlined. It appears however that conclusions are not warranted at this time about the relative effectiveness of the four treatment strategies. Research issues such as subject selection and the need to document that skills acquired during training are associated with relevant outcome measures are discussed. Finally, suggestions for future social skills studies are offered.

In order to empirically investigate the economic development process in rural areas, factors that are associated with the level and changes in employment must be conceptualized, operationalized and measured accurately. This symposium was organized to discuss the conceptualization, operationalization and measurement of these factors.
